A composite semiclosed anaesthetic system suitable for controlled or spontaneous respiration.

چکیده
FOR patients who are breathing spontaneously the Magill attachment or system A (Mapleson, 1954) is the most widely used semiclosed anaesthetic system. A theoretical analysis predicted that if the freshgas inflow were made at least equal to the patient's minute-volume ventilation, then rebreathing would be eliminated (Mapleson, 1954). A similar conclusion has been reached experimentally by various workers. For patients whose respiration is manually controlled, however, a recent study (Waters and Mapleson, 1961) has shown that system D involves less rebreathing than system A. In the course of that investigation, an arrangement was devised which enabled the anaesthetist to change over easily from one to the other. This is now described in the belief that it may have some value in ordinary clinical anaesthesia when it is desired to avoid the use of soda lime.

Will we ever have a universal anaesthetic breathing system.
In this issue of the JOURNAL, Humphrey and his colleagues describe a version of the A.D.E. anaesthetic breathing system 1'2 which they feel is optimal for all patients. Humphrey et al . have shown that the "A.D.E. low-flow universal" system functions as a modified D (Mapleson's classification 3) or Bain circuit for controlled ventilation and, with the switch of a single lever, as a modified Map...
متن کاملComparison of Anesthetic Techniques on Outcomes of Pediatric Rigid Bronchoscopy for Foreign Body Removal
Background Although both methods of spontaneous respiration and controlled ventilation during anesthesia are safe and effective for managing children with foreign body aspiration, there is no consensus from the literature as to which technique is optimal. This study aimed to determine the outcomes of anesthetic techniques in pediatric rigid bronchoscopy for foreign body removal. Materials and M...
متن کاملBlood pressure and cerebrospinal fluid pressure studies during lumbar air encephalography.
Blood pressure and cerebrospinal fluid pressure changes were studied during lumbar air encephalography under general anaesthesia. Two anaesthetic techniques were used. In one group spontaneous respiration with nitrous oxide, oxygen and halothane was used and in the other controlled ventilation with nitrous oxide and oxygen was maintained. Larger falls in blood pressure, on sitting up, were seen...
متن کاملLaparoscopy: blood-gas values and minor sequelae associated with three techniques based on isoflurane.
Three anaesthetic techniques based on isoflurane were compared in outpatients undergoing laparoscopy. Sixty healthy patients were randomly allocated to receive isoflurane via mask (spontaneous respiration), via tracheal tube (spontaneous respiration) or via tracheal tube with controlled ventilation. Moderate hypercarbia occurred in the group breathing from a mask, although there was no further ...
متن کاملAntibacterial action of anaesthetic vapours.
A permanent alternative is to re-site the fresh gas inflow to the back of the absorber where it has always been positioned in the past. With the elimination of the fresh gas flow error, it is found that respirometers placed at the catheter mount and in the circle give almost identical readings with spontaneous respiration. With controlled respiration there is over-reading of the circle respirom...
